Wells Criteria for DVT & Pulmonary Embolism (with Age-Adjusted D-Dimer)
Clinical pre-test probability scoring for Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE), integrated with Age-Adjusted D-Dimer guidance (ADJUST-PE) to safely avoid unnecessary CT imaging.
Wells DVT: Score -2 to 8 (Low ≤ 0, Moderate 1-2, High ≥ 3)
Wells PE: Score 0 to 12.5 (Unlikely ≤ 4, Likely > 4)
Age-Adjusted D-Dimer Cutoff (Age > 50) = Age × 10 µg/L FEU

Diagnostic Interpretation
Wells PE Score is 4.5 (PE Likely). D-Dimer testing is NOT recommended as a negative result cannot safely rule out PE in high-pretest probability. Immediate CT Pulmonary Angiography (CTPA) or V/Q scan is indicated. If hemodynamic instability / shock is present, initiate immediate empiric anticoagulation and bedside echocardiography.

ADJUST-PE validated criteria prevents unnecessary CT radiation in elderly patients.
Clinical Decision Pearls
- The 2-tier Wells system (≤ 4 = Unlikely, > 4 = Likely) combined with age-adjusted D-dimer safely reduces CT scans by ~20% without increasing missed thromboembolic events.
- Never order D-dimer if clinical score is "PE Likely" (> 4), as pre-test probability is too high for a normal result to exclude clot.
Limitations & Confounders
- Unreliable in pregnancy, active severe COVID-19, or post-surgical inpatients with baseline inflammatory D-dimer elevation.
