eGFR CKD-EPI 2021 (Race-Free) & Cockcroft-Gault CrCl
Computes standardized estimated glomerular filtration rate using the official 2021 race-free CKD-EPI creatinine equation, alongside Cockcroft-Gault clearance for pharmacy dosing.
CKD-EPI 2021: eGFR = 142 × min(SCr/κ, 1)^α × max(SCr/κ, 1)^-1.200 × 0.9938^Age × (1.012 if female)
Cockcroft-Gault: CrCl = [(140 - Age) × Weight / (72 × SCr)] × (0.85 if female)

Diagnostic Interpretation
eGFR CKD-EPI 2021 is 71.4 mL/min/1.73m² (KDIGO Stage G2: Mildly Decreased GFR (60–89 mL/min/1.73m²)). Cockcroft-Gault CrCl is 73.8 mL/min for renal medication adjustments.
Calculation Breakdown
Parameters and Coefficients
Sex = Male (κ = 0.9, α = -0.302, multiplier = 1), Age = 55, SCr = 1.2 mg/dL =min(SCr/κ, 1)^α = 1, max(SCr/κ, 1)^-1.2 = 0.708
CKD-EPI 2021 Equation
142 × 1 × 0.708 × 0.9938^55 × 1 =71.4 mL/min/1.73m²
Cockcroft-Gault Creatinine Clearance
[(140 - 55) × 75] / (72 × 1.2) =73.8 mL/min
Clinical Decision Pearls
- The 2021 CKD-EPI equation completely removes race adjustments, as recommended by the joint National Kidney Foundation (NKF) and American Society of Nephrology (ASN) task force.
- CKD requires demonstration of eGFR < 60 mL/min/1.73m² or markers of kidney damage (e.g. albuminuria ACR ≥ 30 mg/g) persisting for > 3 months.
- For narrow therapeutic index medications (e.g. aminoglycosides, vancomycin, DOACs, cisplatin), FDA labeling historically references Cockcroft-Gault CrCl in mL/min.
Limitations & Confounders
- eGFR equations assume stable serum creatinine in steady state. Unreliable in rapidly changing Acute Kidney Injury (AKI).
- Creatinine generation is blunted in severe sarcopenia, amputation, cirrhosis, and vegan diets (falsely overestimating true GFR).
